Acknowledgement page. Upon reception of your donation, we will
send you an acknowledgement letter which can serve as a proof of your donation
with our tax ID and the sequence number of your donation, as well as a user
ID/password, by which you can check every
donation that the Foundation has received and compare with
spending records of the Foundation from the password protected document
section. To make a contribution, please print and fill out this form and mail it with your check/money order. If you are employed in a large organization or company, please check with your employer on their matching incentive/gifts program. Thank you very much! |
