You are welcome to make a contribution in any amount.  You can specify whether to publish your name in our Acknowledgement page. Upon reception of your donation, we will send you an acknowledgement letter which can serve as a proof of your donation with our tax ID and the sequence number of your donation, as well as a user ID/password, by which you can check every donation that the Foundation has received and compare with spending records of the Foundation from the password protected document section.

To make a contribution, please print and fill out this form and mail it with your check/money order.

If you are employed by a large organization or company, please check with your employer on their matching incentive/gifts program.
